Are you running Office 2010 64-bit? If you do not know, open task manager and see if the process has *32 at the end (on 64-bit computer only).

Barcode ActiveX is 32-bit and can't run inside a 64-bit process. If you installed 64-bit Office, you need to change it to the x86 version.
